Woodgate & District Residents Association is partnering with Trilogy Care to deliver aged care coordination services locally in Woodgate
A Memorandum of Understanding with Trilogy Care was signed in September with the intent to become a partner in their Community Managed Partners program.
The collaboration will deliver community-based support to older Woodgate residents with current Support at Home funding.
Trilogy Care’s Community Managed Partners program helps small regional towns to facilitate aged care support within their own community.
The service aims to support people to remain living independently in their own homes for longer as well as providing local employment opportunities.
